Our obligations

Play and Thrive Collective is a paediatric occupational therapy practice. We are committed to protecting the privacy, confidentiality and security of your child’s personal and health information.

We collect, store, use and disclose personal and health information in accordance with the Privacy Act 1988 (Cth), the Australian Privacy Principles (APPs), the Health Records and Information Privacy Act 2002 (NSW) and applicable NDIS privacy and confidentiality requirements.

Parents and guardians may contact Play and Thrive Collective to request access to, or correction of, personal information held about themselves or their child.

Collection and use of information

Play and Thrive Collective collects personal and health information that is reasonably necessary to provide occupational therapy services to your child.

This may include information about your child’s health, development, daily activities, sensory processing, motor skills, self-care, participation, behaviour, education and family circumstances where relevant to their occupational therapy needs.

Information may be used to:

• assess your child’s occupational therapy needs
• develop goals and therapy plans
• provide occupational therapy intervention
• monitor and document progress
• prepare reports and recommendations
• communicate with parents, carers and other professionals involved in your child’s care
• manage appointments, accounts and billing
• meet Medicare, NDIS, insurance or other funding requirements where applicable.

If you do not provide relevant personal or health information, we may not have sufficient information to complete an appropriate assessment or provide suitable occupational therapy services.

Online referral information is collected through the secure Splose form embedded on this website. Splose’s own privacy and data-handling terms also apply when its service is used.

Sharing information

With your consent, Play and Thrive Collective may communicate with other people involved in your child’s care, such as GPs, paediatricians, psychologists, teachers, educators and other allied health professionals.

Information may also be provided to Medicare, the NDIS, plan managers, insurers or other funding bodies where necessary for billing, reporting or service provision.

We will not disclose your child’s personal or health information to third parties for unrelated commercial purposes.

Information may be disclosed without consent where required or permitted by law, including where there are child safety concerns or a serious threat to the health or safety of a person.

Security of information

Your child’s information is stored securely and may be held electronically or, where required, in paper form. Information may also include photographs, videos, audio recordings, assessment results, reports and clinical notes.

Play and Thrive Collective takes reasonable steps to protect personal and health information from loss, misuse, interference, unauthorised access, modification or disclosure.

Access to client information is limited to authorised persons who require the information to provide or administer services.

We also take reasonable steps to ensure that information held about your child is accurate, complete and up to date.

Photography, video and social media

Photographs, video or audio recordings may occasionally be useful for clinical purposes, such as documenting progress, reviewing activities or supporting assessment and intervention.

Consent will be obtained before identifiable photographs, videos or recordings are used for promotional, website, advertising or social media purposes.

Choosing not to consent to the use of photographs or media for promotional or social media purposes will not affect your child’s access to occupational therapy services.

Consent for promotional or social media use may be withdrawn by contacting Play and Thrive Collective. Withdrawal will apply to future use where reasonably practicable.

Telehealth

Where occupational therapy services are provided by telehealth, Play and Thrive Collective will take reasonable steps to maintain your child’s privacy, confidentiality and security.

Parents and guardians are encouraged to participate in telehealth sessions from a private and appropriate environment. Information collected during telehealth appointments will be handled in accordance with this Privacy Policy.

Telehealth sessions will not be recorded without appropriate consent.

Access to and correction of information

You may request access to personal or health information held about you or your child and may request that inaccurate or outdated information be corrected.

Access may be limited or declined where permitted or required by law.

Where parents are separated or there are parenting or court orders in place, requests for information will be considered in accordance with applicable privacy law, parental responsibility arrangements and any relevant court orders. Parents and guardians should advise Play and Thrive Collective of any relevant court orders or restrictions that may affect communication or access to a child’s information.

Privacy concerns and complaints

If you have a question, concern or complaint about how Play and Thrive Collective collects, stores, uses or discloses personal or health information, please contact us so that we can investigate and respond.

If you are not satisfied with our response, you may also contact the relevant external privacy or regulatory body, including the Office of the Australian Information Commissioner (OAIC), Information and Privacy Commission NSW (IPC NSW) or, where relevant to NDIS services, the NDIS Quality and Safeguards Commission.
